Metformin-associated lactic acidosis (MALA) is a rare but serious medical condition that requires swift medical intervention. In a recent trial in Thailand, a new protocol was implemented at Maharat Nakhon Ratchasima Hospital to improve the management of this condition, resulting in a significant reduction in mortality rates and improved treatment times.

Metformin is a common medication used to treat type 2 diabetes, enhancing the body’s sensitivity to insulin and reducing glucose production in the liver. Despite its benefits, it can lead to lactic acidosis in rare cases, especially in patients with kidney or liver issues.

Lactic acidosis is a medical condition where lactic acid accumulates in the blood, causing a sharp decrease in blood pH levels. This can result in muscle weakness, rapid breathing, confusion, and in severe cases, organ failure or death.

The New Protocol at Maharat Nakhon Ratchasima Hospital

The new protocol focused on initiating dialysis immediately upon diagnosing MALA using one of three methods: intermittent dialysis, continuous renal replacement therapy, or peritoneal dialysis.

The study, conducted over five years, showed remarkable results, with the mortality rate at Maharat Nakhon Ratchasima Hospital dropping from 25.7% before the protocol to 13.9% after its implementation. In contrast, the mortality rates at Buriram Hospital, which did not adopt the new protocol, remained nearly unchanged.

Enhancing Medical Team Awareness and Response Times

The protocol not only improved survival rates but also reduced the average time between patient admission and the start of dialysis from 870 minutes to 690 minutes. Additionally, awareness among medical staff about MALA increased from 38.5% to 89.9% after the protocol was implemented.

Dr. Watano Parabiboon, who led the study, emphasized the importance of adopting rapid dialysis pathways for time-sensitive conditions like MALA, highlighting the need for all types of dialysis to be available to ensure flexibility and immediate treatment initiation.
